A urine alarm is a research-supported intervention for bed-wetting. Mowrer [23] studied this type of device in one of the first applied behavioral treatment studies. A urine alarm consists of a small sensor attached to the child’s underwear. The alarm is intended to wake the child upon urination and is thought to work through classical conditioning in which the child begins to associate a full bladder feeling with the need to awaken [24]. According to a review by Gimpel and colleagues [24], “The enuresis alarm may be the most effective treatment for PNE [primary nocturnal enuresis], with estimated success ranging up to 70% …” (p. 26). Additionally, research has found that the urine alarm is much better than medication for attaining long-term effectiveness [25]. Although the urine alarm appears to be the most effective treatment in the long term, there are a number of obstacles to its implementation. Because of the necessary disturbance in sleep when the urine alarm is used, both parents and children must have an adequate level of motivation and patience; otherwise, the treatment may be discontinued too early [25]. An additional advantage of the urine alarm, however, is that it’s relatively inexpensive compared with medication [24].
